When do our dental benefits take effect?
You can begin to enroll in the plan as soon as Aug. 15, 2008. This is a 12-month plan – you can enroll at any time. Once you make your initial $35 enrollment fee payment, you have insurance for 12 month regardless of your employment/student status at MSU. This insurance ONLY covers visits to Midwestern Dental locations.

What is covered under the Midwestern Dental Plan?

How much does the plan cost?
If you are a TA covered under the GEU/MSU contract, your cost is $34.50 a year. Midwestern Dental will bill MSU for the other $34.50.
When do I have to pay my portion?
Payment needs to be made at the time of enrollment. You can pay Midwestern by check or credit card.
Is there a payment plan?
Not for the $35 enrollment fee. The dental provider will work out payment plans with you if necessary. Your bill with the provider needs to be paid upon completion of the treatment.
Where can I get more information?
If you have an appointment for Fall 2005, dental information will be mailed to the address you have on file with the Registrar. If for some reason you do not receive this mailing, you can pick up enrollment forms in the Benefits office at MSU (located in the Nisbet Building) or at the Union office.
